Please submit your Performance Based Bonus (PBB) Form 1.2 (soft and hard copy) using this template (http://deped.in/Form12).
You may check your individual ranking per school with this link (http://deped.in/Form13Elementary & http://deped.in/Form13Secondary)
Deadline of submission of the said report will be on Monday (December 4, 2017) at the Personnel Unit.
NOTE:
* For the Elementary level, please consolidate the soft copy of your reports per district into One (1) CD-RW.
* All hard copies should be signed by the School Head.
* Any correction(s) with the submitted report will NOT be entertained.
